    6-Month Progress Report Form due October 1st, 2011

Reminders

The second check will be requested from Komen Headquarters once your progress report is approved by Komen Iowa (approximate date: Nov 15)

Please be sure you have submitted up-to-date proof of insurance as outlined in your contract

Proper credit should always be given to Komen Iowa on all printed material and must be approved by the Affiliate prior to printing. Please be sure materials read as follows along with the Komen Iowa logo: "Generous funds provided by the Iowa Affiliate of Susan G. Komen for the Cure". Materials for consideration may be emailed to the Grants and Education Coordinator at kdavidson@komeniowa.org.

Grant Change Forms

Komen Iowa Grantees are required to first contact the Grants and Education Coordinator, of any changes related to their funded project. She can be reached at 515-309-0095 or kdavidson@komeniowa.org and should be contacted prior to submitting the paperwork below.

 Please remember, Grant Change Forms requesting a grant extension must be submitted to Komen Iowa at least 60 days prior to the end of the grant cycle. Notification of approval or denial of the request will be made within 30 days of receiving the request.

Educational Materials
